1. Physical exam must have been taken in past 12 months. Appointment slip is acceptable for conditional
admission. Military may take out-of-state physical results and shot record to base/post clinic to have them transcribed
to Hawaii Form 14.
2. Immunizations must be up to date. Signed statement from physician indicating student has begun
vaccination series and is waiting for the next dose in the series is acceptable for conditional admission.
3. TB test must have been taken in past 12 months, at time of registration; be ready to provide the date given, the date read, and the results of the TB test in millimeters.

gas or telephone), or military housing assignment document. Documents must have parent/guardian's name. See above
